Advanced Hyperbaric Recovery can provide the following services FREE OF CHARGE for their patients:
*Written letters presenting your treatment plan, treatment records or treatment results to family members, Physicians and/or insurance providers to aid in financial support of treatments.
*Letters of Medical Necessity signed by the Medical Director.
*Letters stating facts and requesting family support.
| • | Phone calls on your behalf, at your request. –The advocate may assist the patient with back-up calling of the patient’s Physician to expedite referrals, prescriptions or other medical preparation. |
| • | Patient Referrals |
| • | The Patient advocate may also provide initial medical literature to the patient, family or healthcare provider as requested. |
| • | Patient privacy is maintained according to local and national privacy requirements and all of your medical records received are considered privileged. |

| • | Car Service: Transportation each way. Safe, reliable and cost effective. Just $2.00 per mile. |
| • | Liaison service. Providing additional information to Physicians, etc. Only $5.00 per page. |
| • | Case Presentation. $150.00 Per hour. |
| • | Personal support and attendance for Medical, Legal or Social appointments and/or events to make sure your wishes are heard and then carried out appropriately. This is your opportunity to have the support you need as you are healing. $25.00 Per hour. |
